DOT or DMV Physical Exam

The Warner Plaza Medical Clinic facility offers physical examinations for DMV, employment purposes, school athletics, and immigration physicals for Green Card or citizenship applications. The examinations also specialize in DOT and DMV physicals. Tests include vision, hearing, and urine samples.

Our exams are provided at affordable rates and does not require appointments. All walk-ins are welcomed at our treatment facility.

Before scheduling a physical DMV exam, you must make sure you have all the required materials and documents for the doctor. These include a list of current medication and allergies, vitamins, herbal supplements, list of symptoms, results from previous tests, measurements of vital signs, medical and surgical history, and recent physical exam reports.

If you have additional questions you wish to have answered, you are welcome to relay them to our team and doctors. We also recommend providing the contact information of your current general doctor(s).

We are available in three locations throughout Woodland Hills and North Hollywood to provide accommodating, individualized physical exams for multiple purposes. Walk-in patients are encouraged to come to our office, no appointments are necessary.

What forms are required for the DOT Driver Exams?

Certified Medical Examiners must continue using the Medical Examination Report Form and Examiner’s Certificate as found on the Federal Motor Carrier Safety Administration website at These forms and requirements will be valid until December 22, 2015.

Beginning December 22, 2015, Certified Medical Examiners will be required to use the Medical Examination Report (MER) Form, MCSA-5875 and Medical Examiner’s Certificate (MEC) Form, MCSA-5876. The FMCSA will begin updating the CMV Driver Medical Examination Results Form, MCSA-5850, used to report CMV driver examinations through the National Registry.

It will now be a regulatory requirement for Certified Medical Examiners to report to FMCSA on the last business day of the month if the ME has not completed any examinations during the month.
